A preventive visit is a chance to review your health history, talk about habits and goals, and discuss which screenings or immunizations may be appropriate.

  • Review personal and family health history.
  • Discuss blood pressure, cholesterol, and other risk factors when appropriate.
  • Talk through screening and vaccination recommendations for your age and circumstances.
  • Bring up sleep, nutrition, movement, mood, and everyday barriers.

A preventive visit and evaluation of a separate new problem may be billed differently. Ask the office and your insurer what is covered before the visit.

Living with an ongoing condition often means more than a prescription. Follow-up creates space to review how a plan fits your life and whether it needs to change.

  • Discuss common conditions such as high blood pressure, type 2 diabetes, or thyroid concerns.
  • Review medication names, doses, side effects, and refill needs.
  • Understand which monitoring or laboratory tests may be needed.
  • Coordinate information from specialists and discuss referrals when appropriate.

A new, non-emergency symptom or question can be a reason to contact a primary care office. A clinician can determine whether an office visit, another setting, or additional evaluation is appropriate.

  • Describe when the concern began and what has changed.
  • Bring recent test results or relevant care records.
  • Ask what to watch for while waiting for an evaluation.
  • Clarify the plan for follow-up if symptoms persist or change.
