A private primary care relationship should come with a clear explanation of fees, services, access, and responsibilities. Start with these questions.

The agreement should

Ask for a written list of included services, the payment schedule, renewal terms, and how to cancel. Practices use different arrangements; the word “membership” does not tell you what is covered.

  • Is the fee per person, household, month, or year?
  • What is included, and what is excluded?
  • What happens if you move or your physician leaves?
